Filling in a british passport application form . can you help?

the applicants date of birth is 21st april 1953.
there is two blank boxes for the day (21st), then two blank boxes for the month. do i put zero four in these boxes or just leave a blank in the first box and 4 in the second box?
bit confusing. dont want to have it checked at the post office and pay 拢7, just to do it all again.thank you.

Whenever there is 2 boxes for the day....it has to be 0 for the first box if it's below 10, same thing for the month. For the year if it is 2 boxes it is the last 2 digits of the year, i.e 53, but if 4 boxes, it is i.e 1953.

fill it in with dd which would be 04 mm which is ttwo numbers in the month yyyy which is four numbers for the year.get it checked it only charged as a one off payment my daughter took hers back twice because she had signed outside the box and was only charged the once.
