Where do I go to apply for my medicare card in Montreal?


I am on a work permit while waiting for my PR card and am the spouse of a Canadian.

Where is the office located for the application of a medicare card in Montreal?

Its not called Medicare in Canada.it's called a health cards.

you will need to show some id so best to contact them .I know proof of residence is one
photocopies of all pages of your residential lease;
photocopies of all pages of your deed of purchase of residential property;
an employer's attestation confirming that you hold employment in Qu茅bec for longer than six months (the attestation must have been issued not more than 30 days prior to the date the R茅gie receives your application for registration);
an attestation of enrolment in a study program offered at a Qu茅bec educational institution;
a solemn affirmation by the landlord of the dwelling in which you live, or by the tenant who signed the lease, attesting that you live at the address you have provided;
a copy of a bill or of an account statement from a telephone, electricity or cable company showing your name and address, or a copy of a school or municipal tax bill showing the same information, together with your solemn affirmation that you live at the address indicated on the document.

Generally speaking, if you arrive in Qu茅bec from outside Canada, even if you are a Canadian citizen, you will be eligible for the Qu茅bec Health Insurance Plan after a waiting period of up to three months following your registration
The R茅gie will process your application and, if you fulfill the eligibility requirements, will send you a letter indicating when your coverage under the Health Insurance Plan takes effect. You will receive your Health Insurance Card within two weeks after that date.

The R茅gie does not reimburse you for healthcare you receive during the waiting period.

During this period, to save you from having to pay for any healthcare services that you or your family members may need, the R茅gie strongly recommends that you take out private insurance within five days following your arrival in Qu茅bec. Thereafter, coverage is more difficult to obtain. For information about private insurance, contact the Canadian Life and Health Insurance OmbudService (CLHIO).

There's a place in Gal茅rie St-Laurent if you live close to there. Just ask anyone in the mall and they'll gladly tell you where to find the boutique.

At any CLSC or if you have a drivers license at the license bureau.
