How do I fill out this application?

Im unsure of the 7 digit postal code...

xFD City-Ito Harumi-Cho xx-x (this would be your address, this can be done on one line or two lines, it doesn't matter much)

KASUGAI-SHI (this is the CITY, -shi means City)
(AICHI-KEN) (-ken is your prefecture so this would be your STATE)

Fyi, you don't have to write -Shi or -ken and just leave it as Kasugai and Aichi

You don't have your POSTAL CODE there, you might need it. Its one of the most important parts to mailing an item to Japan. That is your address but you are unsure of the postal code?? It should be on your Alien Registration Card, if not the home rental agreement, or some other piece of paper you got to live there. It is usually the item with the capital T with an extra line above it. Example: 銆抶xx-xxxx (you don't have to write the 銆? just the numbers).

I would suggest you verify if that is your address if you are not 100% sure before having items shipped to you.

I`m not Japanese so maybe your should defer to TokyoE (or another Japanese person) with the position of the 52-9. But as far as I knew the **-**-** part of your address is like your house and street name in a western address.
